New Patient Checklist

Once you have scheduled an appointment, you will need to send us the information detailed below. This will ensure that the Breastlink team is fully informed of your situation, allowing time for a pathological review and breast imaging film review prior to your comprehensive consultation. Ideally, these records should arrive 5-7 days prior to your appointment date.

The information required is as follows:

1. Your written medical records

Please send all written records to include:

Breast imaging reports (i.e. mammogram, ultrasound and MRI)

Pathology report from breast biopsy and/or breast surgery

Operative report if surgery has been performed

Most recent lab results

Other imaging reports (i.e. PET, CT, bone scan, MRI, chest x-ray)

Any previous treatment records, if applicable

2. Your pathology slides

Biopsy

Lumpectomy

Mastectomy

Most commonly, a patient's initial diagnosis is confirmed by a needle biopsy of the breast. The tissue removed in the biopsy procedure is prepared on a glass slide(s) and is microscopically evaluated by a pathologist. A similar process takes place when a breast surgery is performed (i.e. lumpectomy, mastectomy). Occasionally, other parts of the body are biopsied when medically indicated. The facility where your procedure was performed can guide you to the corresponding pathology department where your pathology slides are housed (i.e. hospital, outside pathology lab). These pathology slides hold important information regarding your diagnosis. For this reason, Breastlink feels it is a critical component of your 2nd opinion to have your pathology slides reviewed by our breast specializing pathologist.

3. Your Imaging Films

Please send all previous breast imaging films to include:

Mammogram

Ultrasound

MRI

These films provide the physicians with a visual image of your breast health assisting in the decisions regarding your individualized treatment plan.

Additional imaging films to include:

PET, CT, bone scan, non-breast MRI, chest x-ray

The review of these films by our breast specializing team is another essential part of your extensive medical assessment.

If you need to contact a hospital or medical center to get any of the above information released, please print out and use our Medical Release form.

It is advisable to have records delivered in person or shipped by a courier company with shipment tracking service (i.e. FedEx, UPS). Please make sure these records arrive at the designated Breastlink office 5-7 days prior to your scheduled appointment.
